When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Information:

Please remember to bring with you two forms of ID, one proof of identity (photographic ID) and one proof of address.

Proof of identity documents accepted

Photographic ID including full name

We accept one of the following items:

  1. Passport
  2. Driving licence including photograph
  3. Identification card including photograph
  4. Visa including photograph
Proof of address documents accepted

Up to date evidence that you live at the address you have stated on the registration form

We accept one of the following items:

  1. Utility Bill (including mobile phone)
  2. Tenancy agreement
  3. Mortgage statement
  4. Bank statement
  5. Payslip
  6. Driving licence (showing current address)
  7. Official UK benefits paperwork i.e. Job seeker allowance, income support etc.

Please note you can only register for yourself and your children.

If you are homeless within our practice boundary you can register with us or we can refer you to the homeless health service located in the compass centre in Jamaica Street in Bristol.
